Information you provide
The care-request form collects your name and contact numbers, the patient's name and age or age group, a short care summary, requested service, location, preferred schedule and privacy consent. These details are used to review your request and arrange assessment and scheduled care. A client account is not required.
Please share only the initial information needed. Do not submit detailed medical records or put personal or medical information in a page address or URL query string.
Calling or emailing us
The contact links open your phone or email application. They do not submit information through this website. If you contact us, please share only the information needed for your enquiry and avoid sending detailed patient records in an initial email.
Care coordination records
Authenticated administrators can review care requests and record assessments, quotations, payments, nurse assignments and schedules. Patient information is confidential and is not displayed in public nurse profiles. Management is responsible for limiting access to authorized personnel involved in care coordination.
Technical information and logs
The web server may record technical details such as an IP address, requested page, time and browser information as part of normal access or error logging. The application records operational errors to help identify and resolve technical problems.
Application logs are kept outside the public web directory.
Cookies and third-party content
Care-request and administration pages use a session cookie to protect form submissions, display confirmation messages and maintain administrator sign-in. No analytics, advertising trackers, external fonts or embedded third-party media are included.
Retention and access questions
Contact management for information about record retention or to ask about information you have supplied. The retention and deletion policy remains a management configuration item; no fixed retention period is currently claimed.
